Job Description:

We are Looking for “LVN/LPN Long Term Care” for our Facility at Holyoke, MA Location, Please find the below Job Duties and Requirements.

Salary: $35-$40/hr

Responsibilities:

• Planning and managing patient care according to each patient’s needs
• Interviewing patients and recording their medical history
• Obtaining patient vital signs and escorting patients to their rooms
• Administering injections of medications
• Verifying that patient charts are updated promptly and accurately
• Observing and recording patients’ conditions and reactions to medications and treatments

Requirements:

• 2+ Years experience in Long Term Care as LVN/LPN – Required
• Active LVN/LPN License from State of MA
• Active BLS Certificate
